This survey is being conducted through a partnership with the following community organizations: Orange County Health Care Agency, AdvanceOC, The Cambodian Family, Orange County Health Improvement Partnership, Providence St. Joseph Health System, Orange County Black Chamber and Black Chamber Education Fund, United Way, and MECCA (Multi-Ethnic Collaborative of Community Agencies).
